Weaning unit and prolonged mechanical ventilation after critical illness

چکیده
Methods We retrospectively reviewed medical records of tracheostomized and clinically stable ICU adults patients who required PMV (> 21 days) and weaning (> 7 days) and were transferred to a WU or a GW over a 8-year period (2007-2014) after critical illness. The study was carried out in two tertiary care university hospitals. Study variables were age, sex, APACHE II score, principal diagnosis, associated major procedures, length of stay in ICU and out in hospital, TCU and GW, Sabadell score, in-hospital mortality, types of tracheotomy procedure, decision to decannulate and discharge to home or long-care facilities.

Patients characteristics and outcomes of prolonged mechanical ventilation after critical illness transferred to a weaning unit
Introduction Recent trends seem to point out that growing numbers of patients requiring prolonged mechanical ventilation (PMV) and protracted weaning after critical illness with eventually account for a large portion of all Intensive Care Unit (ICU) cost and bed occupancy. Specialized Weaning Unit (WU), may facilitate weaning through focused multidisciplinary expert team, but due to considerabl...
متن کاملTiming of tracheostomy as a determinant of weaning success in critically ill patients: a retrospective study
INTRODUCTION Tracheostomy is frequently performed in critically ill patients for prolonged intubation. However, the optimal timing of tracheostomy, and its impact on weaning from mechanical ventilation and outcomes in critically ill patients who require mechanical ventilation remain controversial. METHODS The medical records of patients who underwent tracheostomy in the medical intensive care...
متن کاملAssociation Between Vitamin D Status and Weaning From Prolonged Mechanical Ventilation in Survivors of Critical Illness.
BACKGROUND In this study, we examined the association between 25-hydroxyvitamin D (25(OH)D) concentration and successful weaning from mechanical ventilation in a cohort of ICU survivors requiring prolonged mechanical ventilation. METHODS This was a retrospective cohort study of ICU survivors admitted to a long-term acute care hospital. Demographic data were extracted from medical records, inc...
متن کاملComparison of the effect of pressure support ventilation and volume assured pressure support ventilation on weaning patients off mechanical ventilation after cardiac surgery
Background: Weaning from mechanical ventilation in the intensive care unit is an important step in the treatment process. More patients in less than 6 hours after cardiac surgery can be separated from mechanical ventilation and extubated. However, 20-40% of patients after cardiac surgery, due to dysfunctional ventilator weaning response (DVWR), still remain under mechanical ventilation. Therefo...
متن کاملSuccessful noninvasive mechanical ventilation in a child on prolonged and life-threatening invasive mechanical ventilation after Fontan operation.
For many decades, the primary treatment of acute respiratory failure has been mechanical ventilation. Intubation and mechanical ventilation carry complications such as upper respiratory system trauma, barotrauma, volutrauma, oxytrauma, atelectrauma, and nosocomial infection, and they increase mortality. Noninvasive mechanical ventilation (NIMV) applied to a specially selected group of patients ...
